En TeQuaidas Diagnostic, ofrecemos servicios móviles de recolección de sangre adaptados a sus necesidades, ya sean a corto, largo plazo o según sus necesidades. Nuestros flebotomistas certificados y asegurados cumplen con las normas HIPAA y OSHA, brindándole un apoyo confiable para cubrir la falta de personal sin el compromiso de contratar personal a tiempo completo. TeQuaidas Diagnostics – Weekly Contract Policy Service Package Overview TeQuaidas Diagnostics offers professional contract phlebotomy services to healthcare facilities and organizations. This package includes: 14 sessions, each consisting of an 8-hour shift, valid within a 1-month period. Certified, compliant phlebotomy services for reliable specimen collection and patient care. Scheduling based on client needs, subject to availability. Pricing & Fees Contract Setup Fee: A $1,300 non-refundable setup fee is required to secure the contract and lock in scheduling. Service Fee: The weekly service package is priced at $2,600. Payment Terms: All payments must be made in advance before services begin. Contract Term & Validity The contract is valid for 1 month from the date of activation. Sessions must be scheduled and used within the contract term; unused sessions will not roll over. Cancellation & Termination The $1,300 setup fee is non-refundable under all circumstances. Cancellation prior to service start: If the client cancels after signing but before the first session, a $500 cancellation fee will be charged in addition to the setup fee. Cancellation after services begin: Client must provide at least 7 days written notice. A 30% fee of the remaining contract balance will apply. Any completed sessions will be billed in full at the agreed rate. No-show or late cancellation (less than 48 hours): Session is considered used and non-refundable. Rescheduling Policy Session rescheduling requests must be made at least 48 hours in advance. Missed or late-canceled sessions will be charged as completed sessions. Agreement This policy ensures that TeQuaidas Diagnostics can provide reliable, professional phlebotomy services while protecting against scheduling and staffing conflicts. By signing this agreement, the client acknowledges and agrees to the terms outlined above.
