By enrolling in a Health Risk Assessment (HRA) plan with TeQuaidas Diagnostics, you agree to the following:

Service Scope: HRA services are for wellness and informational purposes only and do not replace medical advice, diagnosis, or treatment.

Scheduling: Appointments are required. A 24-hour notice is required for cancellations or rescheduling. Missed appointments may be counted as completed visits.

Plan Usage: Services included in your plan must be used within the billing period. Additional visits are billed separately.

Payments: Plans may be billed on a recurring basis. All payments are non-refundable once services begin.

Client Responsibility: You agree to provide accurate information and ensure a safe, accessible service location.

Privacy: We follow HIPAA guidelines to protect your personal and health information.

Liability: TeQuaidas Diagnostics is not responsible for delays, third-party lab processing, or decisions made based on HRA results.

Plan Policy
A 24-hour notice is required for all cancellations or rescheduling requests. Missed or late-canceled appointments may be counted as completed visits.
Visits exceeding your plan limit will be billed separately at standard service rates.
All payments are non-refundable once services have commenced, including unused visits within the billing period.
TeQuaidas Diagnostics reserves the right to pause or terminate services due to non-payment or violation of terms.
