Welcome to the website of TeQuaidas Diagnostics ("Company," "we," "us," or "our"). By accessing our website (https://www.tequaidasdiagnostics.com) and using our mobile phlebotomy, diagnostic support, and I-9 verification services, you ("Client," "User," or "Patient") agree to the following Terms of Service.

These services are currently offered in the Atlanta, GA metropolitan area and surrounding counties.
Medical Disclaimer (Not Medical Advice)
TeQuaidas Diagnostics is a mobile diagnostic support and specimen collection company. We do not provide medical advice, diagnosis, or treatment. Our role is limited to collecting biological specimens and facilitating diagnostic testing ordered by authorized healthcare providers or requested by individuals via direct-pay models. All laboratory results are processed by third-party reference laboratories. You should always consult a qualified healthcare professional to interpret lab results or for any medical concerns. In a medical emergency, call 911 or go to the nearest emergency room immediately. Any health-related information on our website or from our staff is for general information only and does not replace professional medical advice.
Service Appointments and Booking
Appointments must be scheduled through our authorized booking platform. Our primary service area is Metropolitan Atlanta and surrounding Georgia counties. Appointments outside this area may include extra travel fees or be subject to availability. You must provide accurate information, including your current location and valid identification. For mobile services, you are responsible for providing a safe, clean, and well-lit environment for our staff.
Cancellation and Refund Policy
A non-refundable $35.00 cancellation fee applies to any appointment canceled within 24 hours of the scheduled time. If our phlebotomist or representative arrives at the agreed location and you are not present or reachable, the full service fee or the $35 cancellation fee (at our discretion) will be charged. Because of the nature of diagnostic collection, refunds for services already rendered are not permitted.
I-9 Employment Verification
We offer both mobile (in-person) and live video I-9 verification services. The employer and/or employee is responsible for ensuring all original documents are present and valid at the time of verification. We act only as the authorized representative for completing Form I-9. We do not provide legal advice regarding immigration status or employment eligibility.
User Responsibilities and Staff Safety
- Provide a safe, clean, smoke-free, and well-lit environment for our staff.Restrain pets and ensure a non-hostile environment during the visit.Provide true and accurate personal, contact, and insurance information.
Limitation of Liability
TeQuaidas Diagnostics is not responsible for delays in laboratory processing times by third-party labs. To the maximum extent allowed by Georgia law, TeQuaidas Diagnostics is not liable for any indirect, incidental, or consequential damages arising from the use of our services. Our total liability for any claim related to our services is limited to the amount you paid for that specific service.
Governing Law
These Terms of Service are governed by the laws of the State of Georgia, without regard to conflict of law rules. Any legal action must be brought exclusively in the courts located in Atlanta, Georgia.

Your information is used solely for the purpose of providing mobile diagnostics support, specimen collection, and facilitating laboratory testing as ordered by authorized healthcare providers or requested via direct-pay models.

- Personal Identification: Name, date of birth, home address, and contact details (phone/email).Medical Information: Clinical data, insurance details, and specific health information relevant to phlebotomy and diagnostic collection.Employment Data: Necessary documentation (ID, Social Security numbers) for I-9 verification services.Payment Information: Securely processed payment details for service fees and non-refundable deposits.
Disclosure of Information
We do not sell, rent, or trade your personal information. Your PHI is shared only with third-party reference laboratories (CAP/CLIA certified) for clinical testing and with authorized healthcare providers participating in your treatment coordination. We may also disclose information when required by state or federal law, in response to valid legal processes, or for public health activities as permitted under HIPAA. Non-medical information related to I-9 verification is shared only with the designated employer or platform authorized to receive the verification.
Your Rights
Under HIPAA, you have the right to: request access to your health records, request amendments or corrections to your PHI, and request an accounting of disclosures. You may also request reasonable restrictions on how your health information is used or shared for treatment, payment, or healthcare operations.
