Terms and Conditions for TeQuaidas Diagnostics Insurance Health and Wellness Package 1. Introduction Welcome to TeQuaidas Diagnostics. These Terms and Conditions govern your use of our insurance health and wellness package for phlebotomy services. By enrolling in this package, you agree to abide by these terms. 2. Scope of Services TeQuaidas Diagnostics provides phlebotomy services covered under the insurance health and wellness package, including blood sample collection and related services at designated locations or home draws. 3. Eligibility - The package is available to enrolled individuals or groups who meet the eligibility criteria outlined in the enrollment agreement. - Proof of coverage and valid identification must be presented at the time of service. Or Receipts uploaded into the system before arrival and verified. 4. Booking and Appointments - Appointments must be scheduled in advance through our online portal. - Late arrivals may result in appointment rescheduling or service denial. 5. Coverage and Limitations - Coverage is subject to the specific terms of the insurance policy associated with the package. - Some services or tests may not be covered and will be billed separately. - Any maximum limits or exclusions will be clearly specified at enrollment. 6. Payment and Billing - All applicable fees not covered by insurance are payable at the time of service. - Insurance claims will be submitted on your behalf; however, ultimate responsibility for payment rests with the insured individual or group. - Non-payment may result in suspension of services until dues are settled. 7. Data Privacy and Confidentiality - All personal health information will be handled in accordance with applicable privacy laws and regulations. - Data will be used exclusively for service provision, billing, and healthcare-related purposes. 8. Cancellation and Rescheduling - Cancellations should be made at least 48 hours prior to the scheduled appointment. - Rescheduling may be accommodated based on availability. 9. Limitation of Liability - TeQuaidas Diagnostics shall not be liable for any damages resulting from service delays, cancellations, or errors outside our control. - We do not guarantee specific health outcomes from services provided. 10.
