Service Description
Our monthly contract packages provide reliable phlebotomy staffing tailored to healthcare facilities, labs, and wellness programs. All phlebotomists are certified, insured, and compliant with HIPAA, OSHA, and CLIA standards. Each package includes 8-hour shift increments that can be booked as needed within the contract term.
Cancellation Policy
TeQuaidas Diagnostics – Part-Time Monthly Contract Policy Service Package Overview The Part-Time Monthly Phlebotomy Package is designed for healthcare facilities and organizations with moderate patient volume. Services are delivered by certified and insured phlebotomists who adhere to HIPAA, OSHA, and CLIA standards. This package includes: 10 sessions per month, each consisting of an 8-hour shift. Flexible scheduling (approximately 2–3 shifts per week). Professional blood collection, specimen handling, and patient-focused service. Pricing & Fees Setup Fee: $1,300 non-refundable, required to secure scheduling. Monthly Service Fee: $4,500. Three-Month Contract Rate: $12,000 when paid upfront (saves $1,500). Payment Terms: All payments must be made in advance of services. Contract Term & Validity The monthly contract is valid for 30 days from the activation date. Sessions must be scheduled and used within the contract term; unused sessions do not roll over. A 3-month contract option is available with discounted pricing and guaranteed coverage. Cancellation & Termination The $1,300 setup fee is non-refundable. Cancellation before services begin: A $500 cancellation fee applies in addition to the setup fee. Cancellation after services begin: Requires at least 7 days written notice. A 30% fee of the remaining contract balance will apply. Any completed sessions will be billed in full. No-shows or cancellations within 48 hours of a scheduled session are considered used sessions and non-refundable. Rescheduling Policy Rescheduling requests must be made at least 48 hours in advance. Sessions canceled or missed with less than 48 hours’ notice will count as completed. Agreement This policy ensures that TeQuaidas Diagnostics can deliver reliable, professional phlebotomy services while protecting against scheduling conflicts. By entering into this agreement, the Client acknowledges and agrees to the terms outlined above.
